Health Insurance Policy

Health Insurance Requirement Policy

As a condition of enrollment, the University of San Francisco requires regularly enrolled students to have health insurance, both to protect against unexpected high medical costs and to provide access to quality care. The following groups of students are required to have health insurance.

  · All undergraduate domestic students registered for 9 credit hours or more (excluding students in Certificate programs, or Online programs)

  · All graduate domestic students registered for 6 credit hours or more (excluding students in Certificate programs, or Online programs)

  · All international students and scholars registered for at least 1 credit hour or more (including undergraduate, graduate, law, and non­ degree students)

  · All students who reside in University ­operated housing (including undergraduate, graduate, law, and non­ degree students)

University of San Francisco has contracted with Aetna Student Health to provide health insurance for USF students. Students will be automatically enrolled in and billed for Aetna Student Health Insurance Plan when they register for the required number of credits.

Please note: Students who fail to register for the required number of credits by September 1 (Fall semester) and February 1 (Spring semester) by 5 pm will NOT be automatically enrolled and billed for Aetna health insurance. Therefore, it is the student’s responsibility to contact Health Promotion Services within 72 hours to discuss her or his options.

In addition, students who take a leave of absence or withdraw from school and get a full refund (including the insurance premium) will not be enrolled in Aetna Student Health Insurance Plan. These students must also contact HPS within 72 hours to discuss her or his options.

Health Insurance coverage dates

Student coverage will become effective at 12:00 AM on the coverage start date and will terminate at 11:59 PM on the coverage date indicated below:

Annual            August 1 to July 31

Fall Semester        August 1 to December 31

Spring Semester      January 1 to July 31

Summer (students commencing their school in summer)        May 1 to July 31

Coverage is available only to Students enrolling in the health plan and paying premiums. Payment of medical and prescription drug claims incurred by students not fully enrolled in the Aetna Student Health Plan during the coverage period will be the full responsibility of the student. This may include claims that were submitted to, and paid by, Aetna Student Health, if the student later elected to waive coverage, or was not eligible for coverage during the Coverage Period.

All students registered for at least 3 units are eligible to purchase Aetna Student Health Plan.  Student registered below the required units must contact HPS within 72 hours to enroll in Aetna Student Health Plan.

Dependent Coverage

Students’ spouses, domestic partners, or dependent children are not eligible to enroll in the Student Health Plan.

Opting out (waiving) the health insurance Charge

Students who can prove that they have coverage comparable (equal or better) to the University­-sponsored plan under a United States­ domiciled health insurance company may waive this requirement each academic year. Domestic students may waive this requirement online.

International students must waive the University sponsored student health insurance plan with Health Promotion Staff to ensure that their health insurance policy meets the minimum insurance requirement established by University of San Francisco, and United States federal government regulations.

Students are expected to maintain comparable health insurance coverage at all times during their waiver period. However, if a student’s health coverage is dropped, it is the student’s responsibility to contact Health Promotion Services to discuss her or his options.

Waiver Approval


If the waiver is approved, the student’s account will be credited accordingly. However, if a student uses the health insurance, then waives it later or takes a leave, or withdraws from school, the student is responsible for all her/his medical bills.  A continuing student will be recharged the health insurance premium if s/he has outstanding and paid claims with Aetna Student Health.

If a student waives in Fall semester, the waiver is good for Fall and Spring semesters. However, students, who waive in the Spring semester, must waive again for the following Fall semester. Students who have waived the plan in a given year or semester are eligible to re­enroll in the USF­ sponsored health plan for the following semester. These students must contact Health Promotion Service office during enrollment/waiver period to re­enroll in the USF sponsored health plan.

If the waiver is denied, students may file an online waiver appeal form with Health Promotion Services within 72 hours of waiver denial.

Late Waiver Policy

Students who fail to submit a health insurance waiver by the appropriate deadline may file a late waiver appeal request to Health Promotion Services. Incomplete waiver appeal will result in automatic denial. Decision notices will be sent by mail within 3 business days of receiving a complete waiver appeal. If your late waiver appeal is approved a $250.00 late waiver fine will be placed on your student account and you will receive a full refund of the semester health insurance premium.

If your late waiver appeal is denied you will have an opportunity to waive the USF-sponsored insurance plan the following semester.

In order to qualify for a late waiver appeal the effective date of your coverage must be August 1 (Fall semester) and January 1 (Spring semester), and remain in effect for the remainder of the academic year. Late Waiver Appeals will NOT be accepted after the end of September (Fall semester) or the end of February (Spring semester).

Open enrollment/waiver periods

Fall semester           July 15 through September 1

Spring semester     December 1 through February 1

Summer Students

Students who commence their enrollment in the summer session are not automatically enrolled in and not billed for the University­-sponsored health insurance plan. These students must purchase the coverage for the summer session or apply for a waiver in order to comply with the USF health insurance policy.

Optional Practical Training (OPT) Students

International students who are completing their OPT are eligible to voluntarily purchase the student health insurance plan and remain covered for up to 1 year.

If you wish to remain covered under the USF ­sponsored health insurance plan, students must complete the online Health Insurance Enrollment Request Form­ and submit supporting documents. Once the form is completed Health Promotion Services will contact you by email within 72 hours regarding the enrollment process.

Leave of Absence

All Students on an official medical or academic leave of absence are eligible to voluntarily purchase the student health insurance plan and remain covered for up to 1 year. Students must contact the Health Promotion Services office within 72 hours prior to enrolling in the health plan.

If you wish to remain covered under the USF ­sponsored health insurance plan, student must complete the online Health Insurance enrollment request Form­. Once the form is completed Health Promotion Services will contact you by email within 72 hours regarding the enrollment process.

School Withdrawal

All students who withdraw from school are NOT eligible to purchase the school sponsored health insurance plan

Special Program Students

Special programs are short-term programs generally lasting between one to six weeks. Students enrolled in a special program, as well as visiting international scholars, are also eligible to purchase the USF student health insurance policy. Student enrolled in a special program may purchase the plan on a monthly basis (i.e., 8/1/15­-8/30/15). Partial month purchase is not available.
