Parent PLUS Loan Address Update Request Form

This form is to be completed by parent borrowers of a Parent PLUS Loan only and not intended for students. 

Please complete this form to update your mailing address for the Parent PLUS Loan or to authorize a Parent PLUS Loan refund to go to your student. When the refund check is issued, it will be sent to the address you have entered on this form. We will process your request and confirm via email that the address is updated. 

Note: The address update only impacts the Parent PLUS Loan refund.
