Report OPT Employment Form

The Student and Exchange Visitor Program (SEVP), within the Department of Homeland Security, requests that schools ensure the accuracy of certain information in SEVIS (Student and Exchange Visitor Information System) for all students who have requested OPT employment authorization. You are already aware of the requirement to report changes of your residential address within 10 days of any move. This includes the period while you are on OPT.

SEVP requests that schools also collect information about your OPT employment, including your employer’s name and address. As you may know, OPT is defined as employment in a position that is directly related to your major area of study.

Remember that this reporting requirement is an on-going requirement. You must continue reporting to ISSS any changes in your residential address, employment status (e.g., termination of job), and/or the name and address of employer(s). Please report and make changes to your data as often as needed by completing the online form at the provided link below.
