Hi there. In this video I'm going to show you how to request your attendance modification accommodation. Once you've logged into your online portal, you should see an important message that looks like this. If your to do list still says no accommodation requests found that means you still have to request your accommodations through the online portal. If you need any guidance on how to log into your online accounts, please reference the first tutorial video entitled How to request your accommodations. If you scroll down, you should see a screen like this that has your classes listed with a check box next to each one. Check the box next to the class for which you want to request attendance modification and then click step 2, continue to customize your accommodations. In the next screen, it will list your course or courses and have your accommodations listed underneath each course. Make sure to check attendance modification, as well as any other applicable accommodations and then click submit. Once you submit your accommodation request the status of your request will look like this. There will be a blue, question mark, meaning the request is processing it will also show you the date and timestamp of your initial request. Within one business day of doing this, you should receive an email. In this email, there will be a link to a brief questionnaire soliciting information about the attendance policies in each of your courses. The questionnaire is a Google form that looks like this. Remember, you must complete this form for every course in which you are requesting attendance modification. Give us all your contact information. The name of the course, your instructors last name and the name of your specialist. We also ask that you articulate any concerns you might have about the attendance policy or how you see your attendance modification accommodations applying to this course. There's also a question about whether or not you want your specialist to follow up with your faculty. Once we receive this form your faculty will be notified of your accommodations via the faculty notification letter. This question basically gets at whether you want your specialist to reach out, in addition to your faculty notification letter or if you have specific concerns that won't be addressed through your faculty notification letter. Once your Google Form is received by our specialists status of your accommodation request will change to a green checkmark that says approved. You can see that your specialist approved it. And the date and time that it was sent to your faculty over in the notification letter section. Once you have the green checkmark you'll know that your faculty have been notified of your accommodation. If you have any specific concerns or questions, follow up with your specialist or your faculty whomever you feel more comfortable.
