In this video, you will learn how to submit a flex plan after a student requests the attendance modification accommodation in your course. In the faculty notification letter that you receive, you'll be prompted to log in to your SDS online portal and complete the Flex plan. To access the Flex plan, navigate to the Student Disability Services website and select my SDS online services. Once you are logged in. When prompted with this Access Policy screen. Select continue to view student accommodations. On the next page, select the Flex Plan tab on the left hand side of the page. The number of plans not initiated indicates the number of courses in which a student has requested attendance modification, but the flex plan has not yet been completed. To complete a flex plan for a specific course, select a course from the step one select courses section. If you have multiple courses, they will appear in this section. You also have the option to select step two specify flex plan for multiple courses in which your flex plan responses will apply to the courses that you select. Click the check box next to the course in which you'd like to complete the flex plan and select view. If the status says not initiated, that means that the flex plan has not yet been completed. In the Student Requests section, you will see a list of all students that have requested the attendance modification accommodation in that course. Your responses to questions one through 13 will apply to all students listed. After you have answered all questions pertaining to subjects such as the maximum number of additional absences, the procedure for submitting coursework and make up exams during absence, and class participation. Select Submit Flex Plan at the bottom of the page. After submitting, the course will show up in the submitted Flex Plan section of the Flex Plan tab. Completed flex plan questionnaires will say completed in the status section. SDS students in that course will be able to view your Flex Plan questionnaire responses in their SDS portal, and are instructed to contact their SDS specialist if they have any concerns. This shows what it will look like on the student's end. If a student requests attendance modification and the instructor has not completed the flex plan. SDS will send out reminders every three days to the instructor until they have completed the Flex Plan questionnaire. You can copy and reuse your answers from semester to semester if your attendance policies remain the same.
