In this video, you will learn how to view the flex plan submitted by your instructor after you have submitted an accommodation request to apply the attendance modification accommodation to one or more of your courses. After you have requested the attendance modification accommodation, your instructor will be prompted to complete a questionnaire indicating the attendance policy for the course in relation to your attendance modification accommodation. The questionnaire is referred to as the Flex Plan. To view the flex plan, navigate to the Student Disability Services website and log in to my SDS Online Services. Once you have logged into your portal, select the Flex Plan tab within the accommodations menu. Within the Flex Plan tab, you will see a list of your courses in which you have requested the attendance modification accommodation to apply. If the status says completed, it indicates that your instructor has completed the Flex Plan questionnaire. If the status says not initiated, it indicates that your instructor has not yet completed the flex plan. SDS will send out reminders to your instructor every three days until the Flex Plan questionnaire is submitted. Select view next to the course in which you would like to review the flex plan. On this page, you can view your professor's responses to the Flex Plan Questionnaire. To learn the parameters for your attendance modification in the course, your professor will answer the list of questions pertaining to the maximum number of additional absences.
