Please be advised that this page is for reporting non-emergency incidents.

If you have an on-campus emergency, please contact USF Public Safety at (415) 422-2911. If you have an off-campus emergency, please dial 9-1-1.

There are different incident reports available to report various types of incidents occurring on and off campus. Filing any of these incident reports places the university on notice of an incident which occurred. To complete and submit an incident report, please click on the graphic of the appropriate incident report.

Office of Student Conduct General Incident Report Form

This form is used to report most incidents of alleged misconduct. This form is NOT to be used to report alleged violations of the Academic Honor Code or Title IX/sexual misconduct (dating violence, domestic violence, sexual harassment, sexual assault, stalking).
