Short Term Program Forms
All Undergraduate Students must complete the Disciplinary Clearance Form before moving forward with an application. Undergraduate students who do not pass clearance become ineligible to participate.
Domestic OFF-CAMPUS (U.S. Only) Short Term Program Forms
If you have been accepted to an INTERNATIONAL PROGRAM you must complete the mandatory readings and submit all of the following requirements listed below at least SIX WEEKS BEFORE THE SEMESTER ENDS.

Submit original documents (Select ONE waiver form according to your program):
- Disciplinary Clearance Form (Undergraduate students only must complete a Disciplinary Clearance Form prior to beginning the application process. OSCRR will grant disciplinary clearance enabling students to advance in the application process.)
- Waiver of Liability Form - International (submit original document - no scanned copies nor via email allowed)
- Waiver of Liability Form - TW (If traveling to a country with a Level 3 Travel Advisory; submit original document - no scanned copies nor via email allowed)
- Trip Cancellation & Interruption Protection Plan/Terms of Participation Form (submit original document - no scanned copies nor via email allowed)
