USF in DC Semester Program and McCarthy Fellows in Sacramento Program

All USF in DC Program and McCarthy Fellows in Sacramento program participants must complete and submit all of the following compliance requirements at least six weeks before departure.

Note: All USF in DC and Sacramento Fellows are first required to submit a Disciplinary Clearance Form in order to be eligible for the program. This requirement cannot be waived.

Hard Copy Submission to the Study Abroad Office in LM 340 or McCarthy Center

• Waiver of Liability Form (USF in D.C.)
• Waiver of Liability Form (Sacramento Fellows)
