Intake/Eligibility Process
Students with disabilities who are interested in receiving services from Student Disability Services (SDS) can contact our office upon admission to USF, or upon onset of disability to start the 3 step intake and eligibility process. We look forward to talking with you!
All contact with SDS is confidential and disability-related information is not released except on a need to know basis.
Step One: Register
Register with SDS by completing our online application. Students at post-secondary institutions are required to self identify as a person with a disability.
Step Two: Documentation
We encourage students registering with SDS to submit disability documentation and/or information about past accommodations. Students can do this by scanning or taking photos of documentation and uploading it to the online application. Documentation should clearly explain the student’s condition, functional impacts on daily life activities and any recommendations for accommodations. Documentation helps guide the process of determining what accommodations are appropriate for each individual student.
Students are welcome to use our Verification of Disability form while they are collecting documentation.

Step Three: Intake Appointment
Every student with a disability is unique. We want to get to know all of our students. In this appointment, students will meet with a Disability Specialist to have a conversation about your experience and discuss appropriate accommodations and academic adjustments. Your specialist may request additional documentation.
It typically takes 5 to 10 business days after a student submits the online application to be contacted by a specialist to set up an intake appointment (this time may vary during holidays and university breaks).
