  • Add/Drop Form (The pdf of this form is no longer accepted by USF. Please note this link takes you to the registrar's office online forms to complete). This Add form will allow you to request to add a course to your schedule after the last day to add online. The Drop form will allow you to request to add a course to your schedule after the last day to add online.
  • Remote Course Request  (On-line registration starts on November 5th for Intersession, Spring, Summer 2025)
  • Change of Course Units (This form is to request a units change after the course's Census Date. You may use this form only to increase the number of registered units for one of your courses. This form can only be used for courses with variable units.)
  • Change of Academic Program (Please note this link takes you to the Registrar's office list of forms.)
  • Directed Study Registration Form (This online form can only be used by School of Education students and faculty.) 
  • Deadlines to submit the Directed Study form are below:
  •  Fall 2024: Deadline to submit this form is August 19, 2024.
  • Intersession 2025: Deadline to submit this form is November 29, 2024.
  •  Spring 2025: Deadline to submit this form is January 20, 2025.
  • Summer 2025: Deadline to submit this form is May 9, 2025.
  • Fall 2025: Deadline to submit this form is August 18, 2025.
  • Intersession 2026: Deadline to submit this form is November 28, 2025.
  • Spring 2026: Deadline to submit this form is January 23, 2026.
     
  • Incomplete Grade Form (This online form can only be used by School of Education students and faculty.  Faculty, please note: Starting Fall 2021, you will receive an email from the registrar's office reminding you that incomplete grades will turn into F grades without action on your part after one semester.) 
  • Leave of Absence (Please note this link takes you to the Registrar's office list of forms.  You will select "Leave of Absence" and complete the new ONLINE form.  The registrar's office is no longer collecting paper Leave of Absence forms.)
  • Petition to Enroll at Another Institution (The pdf of this form is no longer accepted by USF. Please note this link takes you to the registrar's office online form to complete)
  • Petition for Transfer of Credit (The pdf of this form is no longer accepted by USF.  Please work with your advisor.  Your advisor must submit the substitution/waiver laserfiche form online.)
