PHP Adjunct Faculty Basic Life and AD&D Insurance

Eligibility and coverage:

• This benefit applies to adjunct faculty with PHP status only.
• Eligible adjunct faculty who have a current teaching assignment will be enrolled in Basic Life and Accidental Death & Dismemberment (AD&D) insurance at $40,000. Adjunct faculty age 70+ will be enrolled at $26,000.
• Eligible adjunct faculty will continue to be enrolled in Basic Life and AD&D insurance as long as they continue to have a teaching assignment each semester.
• Eligible adjunct faculty who no longer have a teaching assignment will be disenrolled and will receive a conversion notice from USF’s life insurance vendor, New York Life (NYL).

Important – Remember to designate a Beneficiary(s):

• Each time you become eligible for and are therefore re-enrolled in Basic Life and AD&D insurance, you will need to designate a Beneficiary(s) in USFWorks.
• An existing Beneficiary in USFWorks can be designated by simply selecting the relevant option (Existing Beneficiary Persons) and clicking on their name.
