The Kaiser Mass Vaccination Site hosted at Koret (Swig Gym) is open to Kaiser members AND non-members. If you are not already a Kaiser Permanente member, fill out the eligibility form to request a Medical/Health Record Number (MRN). You can also call 833-574-2273 to request an MRN. You will need an MRN to book an appointment. Once you have a MRN, you can check appointment availability online OR call 866-454-8855 to schedule an appointment.
Effective March 29: California Enacts (Retroactive) 2021 Emergency Supplemental Paid Sick Leave Law
With the passage of Senate Bill 95, 2021 CA COVID-19 Paid Sick Leave, employees who are unable to work or telework can request time off for specified reasons by using the SF PHELO & CA COVID time off type in USFWorks. This new law is retroactive to January 1, 2021 and will for the most part run concurrently with SF PHELO. Please visit the HR myUSF page for more details.
