Basic Life and AD&D Insurance
Life and Accidental Death & Dismemberment (AD&D) Insurance helps protect you and your family's financial security in case of an accident, injury, or death. As an eligible faculty or staff member, you receive basic life and AD&D coverage at no cost to you through CIGNA; USF pays the full premium for both basic life and AD&D insurance.
USF offers Basic Life insurance in the amount of one times your annual salary rounded to the nearest $1,000, with the maximum benefit capped at $500,000. The AD&D benefit has the same coverage level.
Refer to the plan certificate for the low-cost, age-based rates and for the complete schedule of benefits. Evidence of Insurability (EOI) or a health questionnaire may be required if you elect coverage over the guarantee issue amount of $400,000 or if you enroll after your initial eligibility period. Life insurance coverage for faculty and staff, age 70 and older, will be reduced to 65% of the face value.
IMPORTANT: You must designate your beneficiary.
Life insurance beneficiaries can be added or changed at anytime of the year. A beneficiary is the individual(s) who will receive your life insurance benefits should something happen to you. Please review the Managing Life Insurance Beneficiaries to learn more.
Voluntary Life and AD&D Insurance
You may also elect Voluntary Life insurance and Voluntary Accidental Death and Dismemberment (AD&D) insurance, which are employee-paid plans to supplement your USF-provided life insurance.
The cost of voluntary life and AD&D insurance depends on your age and how much coverage you select. Voluntary Life insurance is offered in increments of $10,000, up to the guaranteed issue amount of $400,000, rounded to the nearest $10,000. Voluntary AD&D insurance is offered in increments of $25,000, up to a maximum of $250,000.
