The IRB Process at USF

The general process for students and faculty submitting applications to the IRB is detailed below. Applications must be submitted at least one month prior to the projected start date of the research through the AxiomMentor site

  1. Students work with their faculty advisor to determine if their research qualifies as human subjects research and subject to IRB review. If you are instructed to apply for IRB review, proceed to Step 2. 

  2. To start a new IRB application, log into the AxiomMentor website, click on My Applications and select Create New Application

  3. Download the Application and complete the template form (found in the Application Forms folder). 

  4. Download and create an Informed Consent document

  5. Gather all protocol instruments that you will use with your subjects, including survey and interview questions. 

  6. Complete online AxiomMentor application form and upload the completed IRB Application, Informed Consent and survey/interview question documents to your submission. Submissions missing these documents are considered as incomplete and will not be reviewed until provided. 

  7. After you submit, your faculty advisor will be notified via email and asked to log into the AxiomMentor site to review and approve your application. Student applications will not be reviewed until advisor approval has been received through the AxiomMentor site. Follow up directly with your advisor to ensure they are aware of your application submission. 

  8. Once your faculty advisor has approved your protocol, your application will be reviewed to determine if it is complete. Completed applications will be assigned to an IRB board member for review and a notification will be sent to the applicant. If the application is incomplete, IRB staff will send a notification to the applicant and indicate what will need to be provided before the review can proceed. Missing documentation must be uploaded to the existing application through the AxiomMentor site and not sent via email. Once submitted for review, the review process can take up to 3 weeks. 

  9. Any requested revisions or clarifications by the IRB board member reviewer will be requested through the AxiomMentor site. Once the IRB board member has completed their review, the application will be sent to the IRB Chair for final approval. A final approval notification will be sent out within 2 - 3 business days following completion of review through the AxiomMentor site to the applicant. 

  10. Work on a study cannot be modified from the approved protocol without IRB approval. If any changes are to be made after initial application approval, a Modification Application must be completed, signed by the faculty advisor, and uploaded to the original study application in the AxiomMentor system. 

Once your IRB application has been submitted, you can check the status of your application by logging into the AxiomMentor system and clicking on My Applications. Please direct all communication for your submission by using the Messages link within your application. If you email the IRB with follow up questions, please include your Protocol ID in the Subject Line.
