Please be advised that this page is for reporting non-emergency incidents.
If you have an on-campus emergency, please contact USF Public Safety at (415) 422-2911. If you have an off-campus emergency, please dial 9-1-1.
There are different incident reports available to report various types of incidents occurring on and off campus. Filing any of these incident reports places the university on notice of an incident which occurred.
School of Law Resources

Academic & Honor Code Violations
If you have questions regarding our Academic Misconduct or Honor Code, or if you have a good faith belief that an Academic and/or Honor Code violation has occurred, please contact Associate Dean Amy Flynn via email or call 415.422.6304 to schedule an appointment.
Incidents of Non-academic Student Misconduct
Use this form to report an incident of behavior that may be a violation of the Student Conduct Code. This form is used to report most incidents of alleged misconduct. This form is NOT to be used to report alleged violations of the Academic Honor Code or Title IX/sexual misconduct (dating violence, domestic violence, sexual harassment, sexual assault, stalking). Incidents of Sexual Misconduct
Use this form to report past or present incidents that may be a violation of the Sexual Misconduct Policy (gender-based discrimination or harassment, relationship violence, sexual harassment, sexual assault, stalking). Use this form to report a bias incidents that do not align with the University's mission.
