For items that may involve changes to processes and systems, please use the USF Data Governance Issue Summary form to bring forward an agenda item. Completed forms will be added to the rolling agenda to be scheduled in an upcoming meeting. The summary form should be emailed to Rosana Aguilar 

Data Glossary Template  this form is used to submit data definitions for approval in university reporting. Before the form may be submitted, the form should be reviewed and approved by the Data Owners with their approval to bring forward to the Data Governance Committee to review and approve.  The form may be submitted on Data Assist as an attachment. 

Data Use and Sharing Agreement

Data Use Agreement is a contractual document created for the one-way inbound transfer and use of nonpublic data that is subject to restrictions on its use. Please refer to the checklist to ensure appropriate offices have reviewed and signed off. Questions may be directed to Linda Wong, wonglusfca.edu
