Personal Data Changes
This is to make a change or correct your legal name of record, Date of Birth, or Gender.
Current students: Please complete our Change of Personal Data Web Form. Note: You must be logged into your MyUSF account to complete.
Alumni/Former students: Please complete our Change of Personal Data Form.
In addition to your Change of Personal Data form, we require a Government-Issued Photo ID and one of the following supporting documents:
- Birth Certificate
- Marriage License issued by a county or city clerk
- Divorce Decree from a court of law
- Court ordered name change
- Signed Social Security Card
Social Security Number
To change or correct your Social Security Number, you must provide the following documents:
- Current students: Please complete our Change of Personal Data Web Form. Note: You must be logged into your MyUSF account to complete.
- Alumni/Former students: Please complete our Change of Personal Data Form.
- Government-Issued Photo ID
- Signed Social Security Card
