Tuition Remission Form

Tuition remission forms for the upcoming Intersession/Spring semester will be accepted from Oct. 1-15, 2019. The form will be available starting Oct. 1, 2019.

Please note:

  • Review the tuition remission section on the tuition benefits web page
  • A new, separate form must be submitted for each semester that the tuition remission benefit is requested
  • Additional documentation, listed on the terms and conditions web page, must be submitted to or to Human Resources in Lone Mountain Main 339 if someone other than yourself is applying for the benefit
  • Submitting this form indicates that you have read, understand, and agree to the terms and conditions and the tuition remission plan
