Tuition Remission Form
Tuition remission forms for the upcoming summer semester will be accepted from March 1-15, 2019. The form will appear below starting March 1, 2019.
- Review the tuition remission section on the tuition benefits web page
- A new, separate form must be submitted for each semester that the tuition remission benefit is requested
- Additional documentation, listed on the terms and conditions web page, must be submitted to firstname.lastname@example.org or to Human Resources in Lone Mountain Main 339 if someone other than yourself is applying for the benefit
- Submitting this form indicates that you have read, understand, and agree to the terms and conditions and the tuition remission plan
