Health Requirements for Nursing Programs

The following is a brief overview of the health requirements and account set-up instructions for nursing programs.

Find your program listed below and follow the directions for your program to set up your CastleBranch account and complete your health requirements. Please note that USF requires all students to undergo a background check and drug test.

Undergraduate Nursing Program

Please follow the following steps in order to set up your CastleBranch profile and submit your health requirement documentation before the deadline. Note that you should allow at least two (2) weeks for all requirements to be met and some requirements may take a few months to complete depending on your test results.

  1. Click https://portal.castlebranch.com/uf91
  2. Click Place Order
  3. Click Please Select
  4. Find your program
  5. Click on the order code sent to you via your program administrator. If you have not received an email with your program's code, please contact your program administrator.

Graduate Nursing Programs

Please follow the following steps in order to set up your CastleBranch profile and submit your health requirement documentation before the deadline.   4. Find your program
  5. Please select the package that includes Background Check, Drug Test, and Medical Document Manager.

*Students sponsored by the Kaiser Nurse Scholars Academy (KPNSA) are eligible to complete a Self-Reporting Form.  

**For the RN-MSN and DNP programs, proof of RN licensure will also be required by the deadline.

^For Family Nurse Practitioner DNP (DNP FNP) and Psychiatric Mental Health Nurse Practitioner DNP (DNP PMHNP) programs, an online HIPPA class will need to be completed.
