How to Submit a Modification or Renewal
Once approved, any changes to your protocol, even minor ones, must receive IRB approval before you implement them. Additionally, if you want to continue working on a protocol beyond its expiration date, you must submit a renewal request.
How to Submit a Modification
The application for requesting a modification to your protocol can be found by logging into the AxiomMentor site, opening your Application Page and scrolling down to the bottom of the page to access the Modifications tab. How to Submit a Renewal
Renewal requests are processed through the AxiomMentor site by accessing your existing study and clicking on the Renewals tab located at the bottom of the page. Complete instructions can be found on the How to Submit a Renewal Request to the IRB page.
