Health Insurance Enrollment Request

The following students are eligible to voluntarily purchase the student health insurance plan and remain covered for up to 1 year.

• Optional Practical Training (OPT) students
• Students who have official academic or medical leave of absence
• Students enrolled in at least 3 credits

Requests must be received by Census Date based on the semester you are requesting health insurance. To find out when Census Date will be, please check out the USF Academic Calendar here.

To request health insurance, students must complete the online Health Insurance Enrollment Request Form and attach supporting documents to the form. Once the form is completed our staff will contact you by email within 72 hours regarding the enrollment process.

**Last day to Enroll is February 12 at 5pm.
