Medical Waiver Plan
Opting Out of a USF Medical Plan
If you are not enrolling in a USF Medical Plan (i.e. Anthem PPO/Kaiser HMO) because you and/or your dependents have healthcare coverage elsewhere that is not an “individual market” plan*, you are eligible to enroll in the Medical Waiver Plan in USFWorks and will receive $75/month in taxable income. If your other coverage is considered an “individual market” plan, you are not eligible to enroll in the USF Medical Waiver plan.
*“Individual market plans” include plans sold in Federal or State Exchanges or outside of Exchanges in the individual insurance market.
Annual re-enrollment in the Medical Waiver Plan is required during Open Enrollment.
If you elect to enroll in the Medical Waiver Plan, upon hire, following a Qualifying Event, or during Open Enrollment, you will need to:
- For Medical, select the University of San Francisco Waiver (do not waive it) on the benefit elections task USFWorks.
- Complete a Medical Waiver Attestation task after submitting your elections task.
- For new hire and qualifying event benefit elections tasks, the Medical Waiver Attestation task will appear in your USFWorks Inbox once you have submitted your elections and they have been approved by the Benefits Team.
- For Open Enrollment tasks, the Medical Waiver Attestation task will appear in your USFWorks Inbox once you have submitted your elections.
- If required, you will be asked to complete the San Francisco Healthcare Security Ordinance (SF HCSO) Waiver Form available here and to email the completed form to benefits@usfca.edu.
