How to Submit an Application to the IRB using AxiomMentor
The entire IRB application and review process is conducted online through the IRB AxiomMentor website, with automatic notifications going to all students, faculty and staff involved with a specific application. The IRB board does not provide rush reviews, so be sure to allow sufficient time for processing of your application by submitting at least one month prior to the proposed start date of your research.
Logging into AxiomMentor
The AxiomMentor site uses Single Sign-on, so if you are already logged into MyUSF, you will be taken directly to the site without having to log in again. Once logged into the site, be sure to click on the "IRB" tab. More detailed instructions on how to log in can be found on the Logging into the AxiomMentor Site page.
Entering a New Application
After logging into the AxiomMentor site, click on "My Applications" and then "Create New Application." More detailed instructions can be found on the Instructions for Entering a New IRB Application page.
Submitting Modifications and Renewals
If you need to make a modification to an existing approved application, log into AxiomMentor and locate the Modifications tab at the bottom of your Application Page. Renewals are also completed by clicking on the Renewals tab within your protocol. More detailed instructions can be found on the How to Submit a Modification or Renewal page.
