How to Submit an Application to the IRB using AxiomMentor

The entire IRB application and review process is conducted online through the IRB AxiomMentor website, with automatic notifications going to all students, faculty and staff involved with a specific application. The IRB board does not provide rush reviews, so be sure to allow sufficient time for processing of your application by submitting at least one month prior to the proposed start date of your research.

Logging into AxiomMentor

Follow this link to access the AxiomMentor IRB site. The site uses Single Sign-on, so if you are already logged into MyUSF, you will be taken directly to the site without having to log in again. Once logged into the site, be sure to click on the "IRB" tab. For more information on how to log into AxiomMentor, click here.

Entering a New Application

After logging into the AxiomMentor site, click on "My Applications" and then "Create New Application." All submissions will require you to download and complete the IRB Review of New Research application first. For more detailed instructions, click here.

Submitting Modifications and Renewals
