Activity Waiver Request Form

Recognized student organizations are required to obtain participant waivers for activities that present a risk to participant safety, including sporting and recreational events, physical activities, and other activities involving elevated risk. Submit the Activity Waiver Request Form at least two (2) full academic weeks before the proposed activity date.

Note: For off-campus activities, travel, and off-campus events involving alcohol, submission of the Off-Campus/Travel Notification Form will automatically initiate any required waiver and risk management review. 

Upon review of the Activity Waiver Request Form, the Student Organization Consultant Team, or designee, will contact the student group regarding next steps.

Fundraising Proposal Form

In order to fundraise on campus, student organizations must obtain approval. Submit the Fundraising Proposal Form at least one (1) full academic week prior to the fundraiser event. Fundraisers held on campus and/or involving the sale of food items are subject to additional review and approval by Events Management & Guest Services.  Please review the General Provisions for Fundraising and Student Organization Meeting and Events Guidelines before submitting a proposal.

Upon review of the Fundraising Proposal Form, the Director of Student Leadership and Engagement, or designee, will contact the student group regarding approval or rejection.

Off-Campus Event/Travel Notification Form

Recognized student organizations have the ability to host events and travel off-campus within the United States on behalf of their organization, as long as approval is obtained. Submit the Off-Campus Event and Travel Notification Form at least two (2) full academic weeks before an off-campus event in San Francisco, three (3) full academic weeks before an off-campus event involving alcohol, and five (5) full academic weeks before the anticipated off-campus travel.

Note: No international travel is permitted for student organizations. Events or travel involving a rental agreement, entertainment/speaker agreement, permit application, or other contractual elements may require additional processing time. Student organizations should submit contractual materials at least two (2) full academic months in advance to allow for contract review and approval. 

Upon review of the Off-Campus Event/Travel Notification Form, a Student Leadership and Engagement staff member will contact the student organization regarding next steps.

OFF-CAMPUS EVENT/TRAVEL NOTIFICATION FORM

Speaker/Entertainment Agreement Form

Recognized student organizations have the ability to request outside speakers, entertainers, performers, etc. to participate in their organization events and meetings, as long as approval is obtained. Whether compensated or uncompensated, student organizations must submit the Speaker/Entertainment Agreement Request Form at least two (2) full academic weeks before the event.

Upon review of the Speaker/Entertainment Agreement Request Form, the Assistant Director for Student Organizations, or designee, will contact the student group regarding next steps.
