Immunization

As a condition of enrollment, USF requires immunizations for specific student populations. USF believes immunizations help maintain the long-term health of our campus and the greater University of San Francisco community.

Submit Proof

To submit your documents through Med+Proctor, please select University of San Francisco from the drop down menu and sign-in through SSO using your USF username and password. Then, students will need to create their account by adding personal identifying information, such as name, date of birth, and student ID number (if you do not know your student ID, here are directions on how to find it.). 

  • We will accept: Immunization Yellow card, high school immunization records, or a copy of medical records.

All immunization records must be in English. If you don't have your immunization records or it needs to be translated, please contact our office and we can send you a form to have your medical provider complete the required documentation.

Students who do not have a “verified” status on file at Med+Proctor have not met their immunization requirement. Students must check their USF Dons email account for important communication regarding their status with Med+Proctor.

After registering and completing your free online profile, you may pay an optional one-time fee of $10 for membership benefits, but it is not required. 

Deadlines

The deadline to submit proof of required immunizations depends on your term of admission:

  • Fall Semester: August 15 at 5 p.m.*
  • Spring Semester: January 15 at 5 p.m.*

*Students planning to reside in USF-operated housing must submit all required immunization records prior to their moving in day.

About Holds and Fines

Failure to submit the immunization records by the deadlines will result in a $100 fine and registration hold. To remove a hold or appeal a fine placed, all required documents must be submitted. To appeal the fine, you may submit the HPS immunization fine appeal form. Submitting this appeal will not guarantee that the fine will be removed. Please notify our office if you need support.

Effective Fall 2023 for new and incoming undergraduate students, failure to submit the immunization records by the deadline will result in a registration hold. If you do not submit all required immunization records after your first two semesters, a $100 fine will be placed on your student account.

All Nursing Students

Starting Fall 2026 nursing students must submit their health and immunization records in two separate systems to meet both university and clinical placement requirements.

  • USF Required Immunizations: submit your required university immunization documentation through Med+Proctor to satisfy USF immunization requirements. 
  • School of Nursing and Health Professions (SONHP): submit the additional health and immunization documentation required for clinical placements to Complio, which will replace CastleBranch. 

Please note:

Nursing students are required to meet additional health and immunization requirements beyond the standard USF immunization requirements to comply with clinical placement requirements. The School of Nursing and Health Professions (SONHP) will contact nursing students directly with instructions for completing nursing-specific health requirements and submitting the required documentation. Please follow the nursing health requirements instructions sent by Vandon Nelson, Program Assistant. 

At this time, clinical placement requirements include an up-to-date COVID-19 vaccination. If you have questions about nursing-specific health requirements, please contact nursing@usfca.edu.
