COVID-19 Vaccine

California is allocating COVID-19 vaccines as they become available to the population. Vaccinations are prioritized by age and risk. You can check to see if you are eligible for the vaccine at:

Vaccination Requirements for Return to Campus

The University of San Francisco will require all undergraduate and graduate students, as well as all faculty and staff employees, to be fully vaccinated for COVID-19. The vaccine requirement will go into effect at the start of the fall semester on August 23, 2022. The deadline for new incoming students is August 15, 2022.

As of January 5, 2022, COVID-19 boosters will be required for all students as soon as you are eligible. Students must upload your updated vaccination information to Med Proctor when they receive their booster. If you already received a university-approved vaccination exemption, this new requirement does not apply to you. If you are unable to get a booster prior to returning to USF, you will be able to receive it at a booster clinic on campus or at the USF Student Health Clinic.

If you have received your COVID-19 vaccine, either dose 1 and/or 2, and booster please upload all vaccine results to Med+ProctorPlease note: students only need to provide their USF email, student ID and name to create their account. After registering your information and completing your free online profile, you may choose to pay an optional one-time fee of $10 for membership benefits, but it is not required.

COVID-19 Exemptions

With proper documentation, USF will accept medical exemptions and religious beliefs exemptions to the COVID-19 vaccine requirement and not the booster. Medical exemption requires a letter from a health care provider stating that the immunization is medically contraindicated. Religious beliefs exemptions will only be accepted for the COVID-19 vaccine (and not other USF-required immunizations). An exemption requires a signed written statement from the student describing the reason for the exemption. Statements must be signed and submitted to USF Health Promotion Services.

COVID-19 San Francisco Testing Options

  • For the spring 2022 semester students can receive a molecular rapid antigen test every Monday, Wednesday and Thursday with GoHealth. To participate please complete the following form and bring your a copy of your insurance card and photo ID to the testing location.
    • Thursday testing will be from 9:30-11:30am at UC 5th Floor
    • Friday testing will be from 9:30-11:30am at UC 5th Floor
  • Beginning, Monday, May 23, Summer 2022 COVID-19 testing will be available to all USF community members and they can receive a rapid antigen test on the following dates and times below:
    • Monday: 11am-1pm Drop in clinic at University Center 501
    • Tuesday: 11am-1pm Drop in clinic at University Center 501
    • Wednesday: 11am-1pm Drop in clinic at University Center 501
    • Thursday: 11am-1pm Drop in clinic at University Center 501
    • Friday: 11am-1pm Drop in clinic at University Center 501

COVID-19 Training Modules (EVERFI)

This spring 2022 semester, the "Staying Health in a Changing Environment," EVERFI Course will be required for all new students studying or living on-campus. Students who are in an online program are excluded from taking the course. New students who are registered for the spring 2022 semester will receive an invitation from EVERFI to take the course.

The COVID-19 online course has been designed to provide you with important information and facts about COVID-19, effective strategies to protect yourself and others, and information to prepare you for returning to our USF campus and contributing to making our community as safe as possible.

Students who fail to complete the course by January 24, 2022, will have a registration hold placed on their student account. If you do not receive your invitation, please contact

After you finish the course, we strongly recommend you download a copy of your certificate of completion for your records. 

Dons Health Check

The Dons Health Check survey is required to be completed by all students who are coming to USF campuses. This can be accessed through the myUSF portal. The purpose of this is to keep our onsite population healthy through tracking daily health checks in order to identify any symptoms of illness early on, thereby limiting the viral spread of COVID-19 at USF.
