- MMR (2 doses, one month apart)
- Hepatitis B vaccination or proof of immunity is required for all students age 18 or younger.

- Students in online programs
- Domestic students born before January 1, 1957 are excluded only from the MMR and Hepatitis B requirement.

If you don't have your immunization records and/or they need to be translated, please reach out to our office and we can provide you a form to have your medical provider complete the required documentation.

- Tuberculosis test taken in the last 12 months. Students with a positive TB test are required to submit a result of chest x-ray.

Nursing students must submit their health and immunization records in two separate systems to meet both university and clinical placement requirements.
- USF Required Immunizations: submit the required university immunization documentation to satisfy the USF immunization requirements listed above.
- School of Nursing and Health Professions (SONHP): submit the additional health and immunization documentation required for clinical placements.
USF Required Immunizations

- Graduate domestic nursing students living off-campus.
- Domestic students born before January 1, 1957 are excluded only from the MMR and Hepatitis B requirement.

Nursing Health Requirements
The SONHP requirements are in addition to the university immunization requirements.
Due to immunization and vaccination requirements of clinical sites, nursing students are required to submit additional immunization and vaccination records to SONHP. The SONHP will contact nursing students directly with details about nursing-specific health requirements and submission procedures.
Due to the clinical sites onboarding requirements, the health requirements for nursing students continue to include an up-to-date COVID-19 vaccination. If you have any questions, please contact nursing@usfca.edu.

How to Submit Your Records
To submit your documents through Med+Proctor, please select University of San Francisco from the drop down menu and sign-in through SSO using your USF username and password.
The University of San Francisco no longer requires the COVID-19 vaccine for students. All campus community members are strongly encouraged to stay up-to-date on vaccinations recommended by the U.S. Centers for Disease Control and Prevention. This includes the COVID-19 vaccine, which is proven to be safe and effective in preventing serious illness, hospitalization, and death.
