Take the NCHA Survey

Complete by March 14
USF students are invited to participate in the National College Health Assessment (NCHA), a confidential survey designed to assess student health behaviors and improve campus wellness programs. Organized by the American College Health Association (ACHA) on behalf of the Title IX Office and Health Promotion Services, the survey is open until March 14, 2025.
How to Participate
The online survey takes approximately 20-30 minutes. We recommend completing it in one sitting. To participate, please search for USF NCHA Survey in your USF email inbox. Each email contains a unique link for you to complete the survey.
Incentives
Upon completing the full survey, you'll be entered into a drawing for eight exciting prizes. Two winners will receive $50 flexi gift cards from the Office of Community Living, five winners will receive $100 gift cards to the USF Bookstore (also valid at Barnes & Noble), and one lucky winner will receive two tickets to see the Golden State Warriors at the Chase Center.
Confidentiality & Support
Participation is voluntary and confidential. No identifiable data will be shared with USF. There may be some personal discomfort with the content of certain questions, regarding illegal substance use, interpersonal violence, and sexual behavior.
