3 Day Food Record Form

If you scheduled a nutrition education appointment, please fill out the 3-day food record form below. Record everything you eat and drink for three days (if possible, 2 weekdays and 1 weekend day). Try to choose days which reflect your normal eating habits, and include what time the food was eaten.
