Medical

For medical coverage, USF offers a choice of a Health Maintenance Organization (HMO) through Kaiser Permanente or a Preferred Provider Organization (PPO) through Anthem Blue Cross. Refer to the HR Forms and Compliance Information web pages for a summary of benefits, evidence of coverage, and claim forms for each provider.

USFFA Faculty Medical Coverage

USFFA faculty medical coverage starts on the date of hire and is determined by the faculty member's duration of their teaching assignment and tenure status. Note: Moving from full-time to adjunct status will affect benefits coverage.

Opting Out of a USF Medical Plan: Medical Waiver Plan Information

If you are not enrolling in a USF Medical Plan (i.e. Anthem PPO/Kaiser HMO) because you and/or your dependents have healthcare coverage elsewhere that is not an “individual market” plan*, you are eligible to enroll in the Medical Waiver Plan in USFWorks and will receive $75/month in taxable income. If your other coverage is considered an “individual market” plan, you are not eligible to enroll in the USF Medical Waiver plan.

*“Individual market plans” include plans sold in Federal or State Exchanges or outside of Exchanges in the individual insurance market.

Annual re-enrollment in the Medical Waiver Plan is required during Open Enrollment.

If you elect to enroll in the Medical Waiver Plan, upon hire, following a Qualifying Event, or during Open Enrollment, you will need to:

  1. For Medical, select the University of San Francisco Waiver (do not waive it) on the benefit elections task USFWorks.
  2. Complete a Medical Waiver Attestation task after submitting your elections task.
    • For new hire and qualifying event benefit elections tasks, the Medical Waiver Attestation task will appear in your USFWorks Inbox once you have submitted your elections and they have been approved by the Benefits Team.
    • For Open Enrollment tasks, the Medical Waiver Attestation task will appear in your USFWorks Inbox once you have submitted your elections.
  3. If required, you will be asked to complete and return the San Francisco Healthcare Security Ordinance (SF HCSO) Waiver Form available at https://myusf.usfca.edu/human-resources/forms and to email the completed form to  benefits@usfca.edu.

USF offers a dental PPO plan through Delta Dental of California, giving you the freedom to visit any in-network or out-of-network dentist you choose. A higher out of pocket expense to the employee may apply if the dentist is not a Delta Participating Dentist.

This incentive plan rewards employees for going to the dentist at least once a year. During the first year, the Delta Dental plan provides 70% of reasonable and customary charges on diagnostic, preventive, and basic dental work up to a maximum of $1,500 per year per participant. Coverage increases by 10% each year after until the fourth year when it reaches 100% if the benefit is used in each prior year. USF pays the full premium for employees and eligible dependents.

USF offers vision benefits through Vision Service Plan (VSP). The benefits of this voluntary plan include an annual eye exam for $20 and a capped allowance for frames and contact lenses every two years at participating providers. Premiums are subsidized and paid through payroll deductions.

Is Your Healthcare Provider In-Network?

Is your healthcare provider in-network? We recommend confirming on a regular basis with your providers that they are still in-network so that you pay the lowest costs (this includes medical, dental and vision). If you go out of network, the provider can charge more than the amounts allowed by the plan. The provider may bill you for the remaining balance. For coverage information, please refer to the Benefits Guide and summary of benefits documents on the Benefits Compliance Information page. You can also find network directories on your plan’s website and contact information if you would like to contact the vendor by phone or email.


San Francisco Healthcare Security Ordinance

USF employees who are not classified as full-time may be eligible for a health care expenditure under the San Francisco Health Care Security Ordinance (SFHSCO).

If you live or work outside of the State of California, it is important that you contact the Benefits Team at benefits@usfca.edu or call at 415-422-2442 to check if there is an impact on your Benefits.

Kaiser Permanente is not offered for employees living outside of California.
